Sam hit the ground running to manage a commissioning continuing healthcare team. Sam implemented team and role redesign, developed shared pathways and joint working with the Local Authority, improved processes, developed a quality assurance framework and several service specifications, all in a very short period of time. Sam was able to build relationships with all the key stakeholders and the team quickly in order to be fully effective in this interim management role. Sam also led on the retrospective application process and the implementation of personal health budgets. This has occurred alongside the management of a complex and challenging service.
Support to Families
Sam has supported families to navigate and understand health and social care systems in order to manage their elderly parents. Sam has supported them through the continuing healthcare process for their relative and offered case management support. Sam has assisted the facilitation of hospital discharge and in securing a placement within a care home.
Advanced Practice Course for Clinicians
Sam worked with a community organisation to develop a comprehensive geriatric assessment course for their advanced practice clinicians to undertake. Sam was in the unique position of having the advanced practice clinical knowledge from her background of working with older people and the leadership and management skills to take this programme forward in a timely way.
Sam was asked to review a continuing healthcare service which was performing poorly and in financial difficulty. Sam was able to rapidly analyse the service, performance issues and propose recommendations for urgent improvement. She also completed a skill mix review and provided recommendations for investment.
Sam developed a competency framework for unregistered and registered nurses and allied health professionals working in the primary care setting. This framework was developed to ensure that appropriate levels of competency in core skills were able to be demonstrated. Sam also developed an advanced practice competency framework for those working in autonomous clinical advanced practice roles.
In previous NHS roles Sam has compiled organisational quality accounts and coordinated the actions relating to this. Sam has independently supported an organisation with the development of their 2012/13 quality account.
Tendering for new business support
Sam has supported a provider in their application to tender for a new service. This provider used her extensive knowledge of a particular service in order to inform the application process.
Professional Practice Investigations and Root, Cause, Analysis (RCA) completion
Sam has performed various investigations and RCA’s, many of which surrounded concerns over professional practice and conduct. Others have related to a serious incident or patient death.
Professional investigations regarding registered nurses performance
Sam has performed independent reviews of the professional management of a service, and investigated the conduct of registered nurses where concerns have been raised.
Sam acts as an expert nurse assessor for continuing healthcare. She is an expert in performing continuing healthcare assessments, checklists, full decision support tool completion and retrospective needs portrayal documents.
Expert Witness Reports
Sam has prepared many expert witness reports for solicitors on a range of clinical issues e.g. standard of care to a patient in a care home or in the community setting. Sam has also provided reports for the Nursing and Midwifery Council (NMC) which have been used in fitness to practice investigations. Sam has also been instructed to provide reports by the Coroner. Sam is experienced in giving verbal evidence in court too.
Care Home Support Team
Sam set up a care home support team for a Clinical Commissioning Group at short notice with limited resources. This was very successful and as such a business case was developed and a substantive service was commissioned. Sam also complied a comprehensive service directory for care home registered nurses.
Domestic Homicide Review (DHR)
Sam has authored many DHR reports on behalf of a Clinical Commissioning Group. Sam is able to produce a chronology of events and analyse complex information to formulate a robust report with clear conclusions.
Chairing Continuing Healthcare Panels
Sam is competent and experienced in chairing continuing healthcare panels. Sam has acted as an independent chair for appeal panels around England. Sam is fully conversant in the National Framework for continuing healthcare.
